What Can I Expect?

Whilst abortion advocates are celebrating the wider availability of this drug to women in Australia, it is clear that the use of drugs for a medical abortion is not a simple, painless solution for the majority of women. You can expect significant cramping and bleeding which will last an average of 9-16 days, with some women reporting bleeding lasting a month or more. Pain has been described by women as anything from ‘uncomfortable’ to ‘excruciating’. You may see the embryo or foetus expelled from your body. This can be very distressing for some women. For up to 8% of women a surgical procedure may still be required in order to stop bleeding or because the medical abortion did not work and the woman is still pregnant.

Mifepristone

Mifepristone acts as an anti-hormone drug that helps to stop the effects of progesterone. It is used for the therapeutic execution of pregnancy. Mifepristone is used to end a pregnancy that is less than 49 days in duration. It works by stopping the supply of hormones that maintains the interior of the uterus. Without these hormones, the uterus cannot support the pregnancy and the contents of the uterus are expelled. Mifepristone is also used to control high blood sugar (hyperglycemia) in patients with Cushing's syndrome who also have type 2 diabetes and have failed surgery or are not candidates for surgery. For purposes of this treatment, pregnancy is dated from the first day of the last menstrual period in a presumed 28 days cycle with ovulation occurring at mid-cycle. It is advisable to go for a clinical examination to know the duration of pregnancy, this may be determined from menstrual history as well. Medical abortions can be done through 9 weeks of pregnancy. Up to 7 weeks, the combination of mifepristone (orally) and misoprostol (Vaginaly) is about 92% effective. As the length of pregnancy increases, the effectiveness of mifepristone and misoprostol decreases slightly. Within 4 hours of taking the second medicine (misoprostol), many women have vaginal bleeding and cramping, and the pregnancy is terminated. Most pregnancies end within the first 24 hours after the misoprostol dose. If not, then typically a second dose of misoprostol is given. Misoprostol can be given orally, buccally (dissolved between the gums and cheek), or vaginally. Misoprostol is slightly more effective when given vaginally than when taken by mouth. But many doctors are only giving misoprostol by mouth because of reports of a rare, fatal infection that affected a few women after using vaginal misoprostol. Some studies have shown that taking misoprostol buccally works as well as when it is given vaginally or orally.

Misoprostol

Misoprostol is a drug that is prescribed to prevent stomach ulcers in people who use NSAIDs. Cytotec is a man-made (synthetic) prostaglandin that is used to lessen the jeopardy of abdomen ulcer in patients treated with nonsteroidal antiinflammatory drugs NSAIDs, for example, aspirin, ibuprofen, etc. Cytotec tablets are used for pain and assorted inflammatory conditions, for example, arthritis. Primarily Cytotec tablets is used in patients at high risk for stomach ulcers when treated with NSAIDs, for example, the elderly, patients with attendant devastating diseases, and patients with a history of ulcers. Prostaglandins are chemicals that are made within many organs of the body including the abdomen. In the abdomen, prostaglandins are believed to protect the inside layer of the abdomen from the ulcer-producing effects of NSAIDs. Scientists now believe that NSAIDs produce ulceration by preventing the creation of prostaglandins in the abdomen. Man-made prostaglandins such as Cytotec given orally replaces the prostaglandins whose production is inhibited by NSAIDs and have been shown to protect the lining of the stomach from NSAID-induced ulcers. Cytotec was approved by the FDA in December 1988. The manufacturers warn against its use in pregnancy and have issued warnings about its use in medical abortion as the safety of the drug in this setting is uncertain.

In a medical abortion misoprostol is used to cause uterine contractions in an attempt to expel the embryo from the woman’s body. It is taken 24-48 hours following the mifepristone.
